Please submit only one of these upload submissions per group with all necessary forms for everyone in your booth. We suggest nominating your most responsible member in the party. Based on this information you will be approved or denied to tattoo by the Department of Health. Once the correct information is submitted, you will be invoiced for your permitting fees. Below are the links to the three forms you will need to fill out, sign, & scan for return. These forms, BBP, and IDs can be uploaded at the bottom of this page. All necessary permitting information should be submitted no later than June 18, 2023.
